To change your Name on your Passport, we will help you get started in the right direction. Don't be overwhelmed. We are here to assist you each step of the way.

First, you will need to gather all of the required information (listed below) and then send your package to us via Federal Express. You will need to complete the DS-82. You will also need to complete and provide an LOA (stands for Letter Of Authorization... it authorizes us to process your passport paperwork). Then download and complete the PassportMax Order Form with payment information and fax it to us at: (512) 469-5951.

Complete, download & print the DS-82 Form.

Complete our LOA (Letter Of Authorization).

The Letter Of Authorization simply enables us to act in the capacity necessary to resolve and answer any problems that may arise, to submit your passport application and to pick up your passport (and any related documents) when it has been issued by the Passport Agency. Get LOA here. * IMPORTANT: Must print and complete 2 copies of the LOA.

We need your existing Passport.

It must be signed.

We need 2 passport photographs.

Must be recent, identical and 2x2 inches.

We need a copy of your travel itinerary.

A copy of either your airline ticket(s) or flight itinerary will serve as the required proof regarding your departure date.

We need an original document stating why your name has changed.

Such as: a marriage certificate, divorce decree, adoption papers or Court Order. The name that you want to have on your passport needs to be reflected in the documents stating your name change. All these documents will be returned with your passport.

We need your $170 check made payable to the U.S. Dept. of State

This is the government fee. Your payment to us is for expediting of the processing of your paperwork, but the Federal Government will still require its standard fees for Lost or Stolen Passport Replacement. NOTE: Please indicate your Date of Birth in the memo field of your check.

Download and complete the PassportMax Order Form with your payment information and fax it to: (512) 469-5951. Payment is $250.00 for 24 hours, $200.00 for 2-5 days, $100.00 for 6-11 days, $60.00 for 12-15 days. We accept American Express, Visa, MasterCard, Discover or payment by check.
