We will help you get started in the right direction in obtaining a Passport for your Child. Don't be
overwhelmed. We are here to assist you each step of the way.
First, you will need to gather all of the required information (listed below) and then take
them to the nearest Acceptance Agent to
seal your documents and then send your package to us via Federal Express. You will need to complete
two government forms: the DS-11 form, and the
DS-3053 form,
which must be accompanied by a copy of the non-attending parents' driver's license (both front and back).
The LOA (Letter Of Authorization) form
authorizes us to process your passport paperwork.
Then download and complete the PassportMax Order Form with payment information and fax it to us at: (512) 469-5951.
To qualify, child must be 15 or younger.

Complete, download & print the DS-11 form from the government website.

Click “Continue” beneath the option that reads “Apply for a Passport Book”. NOTE: Do NOT sign the application form until a Passport Acceptance Agent instructs you to do so. This form should go inside the envelope from the Acceptance Agent.
Complete, download & print the DS-3053 form from the government website.
Complete our LOA (Letter Of Authorization).
The Letter Of Authorization simply enables us to act in the capacity necessary to resolve and answer any problems that may arise, to submit your passport application and to pick up your passport (and any related documents) when it has been issued by the Passport Agency. Get LOA here. NOTE: BOTH Parents must sign this for the minor applicant. ALSO IMPORTANT: Must print and complete 2 copies of the LOA.
We need 2 passport photographs.
Must be recent, identical and 2x2 inches.
We need a copy of your travel itinerary.
A copy of either your airline ticket(s) or flight itinerary will serve as the required proof regarding your departure date.
We need a proof of your U.S. citizenship.
This would be one of the following: ORIGINAL certified copy of U.S. Birth Certificate; or a previous valid U.S. passport; or Report of Birth Abroad (Form FS-240); or Certification of Birth Abroad (Form DS-1350); or Certificate of Citizenship or Naturalization from USCIS. NOTE: A certified birth certificate has a registrar’s raised, embossed, impressed or multicolored seal, registrar’s signature and the date the certificate was filed with the registrar’s office; which must be within 1 year of your birth. NOTE: If the child has had a previous Passport, please submit this document, as well.
We need your $140 check made payable to the U.S. Dept. of State, as well as a $25 CASH execution fee to the Acceptance Agent.
These are government fees. Your payment to PassportMax® is for the expediting of your passport processing, but the Federal Government still requires their standard fees for a New Child Passport. NOTE: Please indicate your child's Date of Birth in the memo field of your check.

Payment is $250.00 for 24 hours, $200.00 for 2-5 days, $100.00 for 6-11 days, $60.00 for 12-15 days. We accept American Express, Visa, MasterCard, Discover or payment by check.

For 24-Hour RUSH Service, package MUST be Fed-Ex'ed (using Fed-Ex First 8:30 a.m. delivery).
